The use of pedicled buccal fat pad combined with sequestrectomy in bisphosphonate-related osteonecrosis of the maxilla

چکیده
The use of pedicled buccal fat pad flap (BFP) has proved of value for the closure of oroantral and oronasal communications and is a well-established tool in oral and maxillofacial surgery. Otherwise, the perceived limitations of surgical therapy for bisphosphonate-related osteonecrosis of the jaws (BRONJ) have been widely discussed, and recommendations have largely been made to offer aggressive surgery only to stage 3 patients refractary to conservative management. Oroantral communication may be a common complication after sequestrectomy and bone debridement in upper maxillary BRONJ. We report a case series of stage 3 recalcitrant maxillary BRONJ surgically treated with extensive sequestrectomy and first reconstruction using pedicled BFP. All the cases presented an uneventful postoperative healing was uneventful without dehiscence, infection, necrosis or oroantral communication. We postulate that managing initially the site with BFP and primary closure may ensure a sufficient blood supply and adequate protection for an effective bone-healing response to occur. This technique may represent a mechanic protection and an abundant source of adipose-derived adult stem cells after debridement in upper maxillary BRONJ. We evaluate in this work results, advantages and indications of this technique.

Surgical treatment of bisphosphonate-related osteonecrosis of the jaws with the use of buccal fat pad: case report.
Bisphosphonate-related osteonecrosis of the jaws (BROJ) has been described since 2003 as an adverse effect of bisphosphonate medications. These drugs act on the vasculature and bone remodeling, mainly on osteoclastic activity and can cause areas of necrotic bone exposure. Treatment for the BROJ is not yet defined, but surgical treatment is one of the forms proposed, which may cause oral deformi...
متن کاملBuccal fat pad: A location for obtaining adipose divide stem cells
Background Buccal fat pad is known as anatomic structure in between the masticatory muscles that separate these muscles from each other and surrounding bone. Ease of access, through the intraoral route makes it suitable choice for obtaining adipose derived stem cells. Methods Intraoral incisions for getting access to the buccal fat of pad are; Retromolar incision, incision in medial of pt...

and masseter muscle. While, the mean volume of the buccal fat pat is approximately 10 ml, the main physiological function of the buccal fat pad is to fill the masticatory space and act as a cushion for the masticatory muscles. There have been studies on the use of the pedicled buccal fat pad for closure of oroantral or oronasal communications secondary to exodontias and for postsurgical oral de...
متن کاملRepair of the perforated sinus membrane with buccal fat pad during sinus augmentation.
Several reports demonstrate successful use of the buccal fat pad (BFP) as pedicled graft in reconstructing small to medium sized maxillary defects. BFP harvesting has so far been shown to be an easy, well-tolerated, and uncomplicated technique for oral reconstruction. This case report proposes the use of BFP for repairing of the perforated sinus membrane during sinus augmentation.

PURPOSE To examine the use of the buccal fat pad (BFP) for correction of maxillary osseous defects, preliminary to dental implant reconstructions, and to present patients in whom this technique was used. MATERIALS AND METHODS The blood supply of the BFP was investigated with a special laser Doppler flowmetry fiberoptic probe in situ before herniation and after placement of the pedicled BFP ov...
